New Patient Forms
Please help us be prepared for your first appointment by printing and completing any new patient form that pertains to you or your family member.
Please bring the completed forms to your first appointment at our office.

Office Hours
Monday: 10:00am - 7:00pm
Tuesday & Wednesday: 8:00am - 5:00pm
Thursday: 7:00am - 3:00pm
Saturday*: 8:00am - 12:00pm
*One Saturday per month - During the school year
Regular Appointments
The appointment when you get your braces on is usually 80 minutes in length. You will then be seen every 8-10
weeks for an adjustment which takes between 15 to 45 minutes. If you have a loose, missing or broken wire or brace,
Please call our office as soon as possible, even if your appointment is the same day. We will need to adjust your appointment, if possible, to include extra time to repair anything loose or broken.
Patients are seen by appointment only. We make every effort to be on time for your appoinment and ask that you extend the same courtesy.
If you need to cancel an appointment, please contact the office as soon as possible. Cancel only if it is an emergency. It is difficult to reschedule
a missed or cancelled appointment since most appointments are made 8 to 10 weeks in advance.
Treatment Fee Arrangements
The cost of treatment depends on the severity of the patient's orthodontic problems. You will be able to discuss fees and payment
options before treatment begins. We have payment plans to suit different budgets, including a no-down
payment option for qualifying patients. We accept VISA and Mastercard. We also accept assignment from most insurance plans, and file the necessary paper work to the insurance company.
Insurance
If you have insurance, we will help you determine the coverage you have available. We ask that you assign your insurance benfit to us.
The balance will be arranged for you to pay.
We will assist you in every way possible in filing your claim and handling insurance questions from our office on your behalf
